what the record holds for a purchaser
  • The carrier’s published roster and machine-readable rate file as they stood, by vintage — a dated record for the plan or board file.
  • Each roster entry screened against federal NPPES deactivation and OIG exclusion records — matched on identifiers wherever one exists; name evidence surfaces candidate records only and never asserts a match by itself.
  • State-licensing (roster licensure) screen: suspended as of 2026-08-04. The licence-binding method behind it produced findings we have withdrawn in full; it is not offered, not run, and not counted until its stated remediation is complete.
  • What changed across captured editions where repeated coverage exists — and, where it does not, that fact stated rather than a change implied.
  • Where captured sources disagree, the disagreement is preserved rather than silently resolved to one answer.
  • Every figure sealed and independently checkable — your consultant can check the whole thing without asking us.
